Client Questionnaire
We value your opinion on your experiences with us. We would like to complete a questionnaire about your perceptions of the services or supports you have received from our agency. This information is being collected to help us identify areas of strengths on which to build, and areas for improvement. If you have a formal complaint or concern we ask that you submit this through our website (https://spotless-quartz.flywheelstaging.com/client-concern-complaint). The survey takes most people between 15 and 20 minutes to complete, but there is no time limit.
